From 1 July 2025, Medicare has updated the Chronic Disease Management program:
The previous GPMP (General Practitioner Management Plan) and TCA (Team Care Arrangement) are now referred to as the GP Chronic Condition Management Plan (GPCCMP).
Existing GPMPs and TCAs remain valid until 30 June 2027.
You must have had a planning or review appointment with your GP in the past 18 months to be eligible.
Eligible patients with a valid GPCCMP referral may access Medicare rebates for the following allied health services at Wholistic Health Centre:

You have a chronic medical condition that has lasted 6 months or longer.
Your GP has prepared a GPCCMP referral for allied health treatment.
You haven’t already used your 5 subsidised visits for the current calendar year.
